* Flatten repo layout to match cross-agent skills convention
Move skills from plugins/entire/skills to top-level skills/ and lift
plugin manifests to the repo root so Cursor, Codex, and other flat
auto-discovery installs work via git clone ... ~/.<agent>/skills/entire.

* Bump to 0.2.0 for flattened layout and drop stale GEMINI.md reference
Signals the repo-layout change to pinned installers, and removes the
contextFileName field from gemini-extension.json now that GEMINI.md is gone.
