refactor: rename to session-handoff, rebrand as agent-plugins

Rename skill from hand-off-session to session-handoff, update plugin branding from "Entire Agent Skills" to "Agent Plugins" with "Entire Inc" as author, simplify install docs with clone+symlink instructions, and remove superseded plan docs and benchmark script.
